About the NDIS

The National Disability Insurance Scheme (NDIS) is Australia's national system for funding support and services for people with permanent and significant disability. It provides eligible participants with a personalised funding plan to access the supports they need.

To be eligible you generally need to be under 65, be an Australian citizen or permanent resident, and have a permanent disability that significantly affects your ability to participate in everyday activities. An early intervention pathway also exists for certain conditions.

You can apply by calling the NDIS on 1800 800 110 or contacting a local Area Coordinator. You will complete an Access Request Form and provide supporting evidence from a treating health professional.
Your plan includes your goals, your funded support categories (Core, Capital, and Capacity Building), and the amount allocated to each. Plans are reviewed regularly or when your circumstances change significantly.

About Pacific Home Care

Yes. Pacific Homecare is a registered NDIS provider, meaning we meet the standards set by the NDIS Quality and Safeguards Commission. You can use our services if your plan is NDIA-managed or plan-managed.
We provide NDIS support services in Hobart (TAS), and Melbourne (VIC). Contact us to confirm service availability in your specific area.

We offer SIL, STA (Respite Care), Community Access, Community Nursing, ILO, Complex Care, Mental Health Support, In-Home Care, and Transport Services.

Yes. You have the right to change providers at any time. We can guide you through the process of transitioning your supports from your current provider to Pacific Homecare.

Getting Started

No. While a support coordinator can be very helpful, you do not need one to contact or access our services directly.
Start times depend on the support type, your NDIS plan, and our current availability. We will always give you an honest timeframe when you first contact us.
It helps to have your NDIS plan reference number, but you do not need everything worked out before reaching out — we are happy to help you figure it out.

About Your Supports

Yes. We take your preferences seriously when matching — including cultural background, language, gender, and interests. While we cannot guarantee every preference, we always work hard to find the right match.
We will notify you as early as possible and arrange a suitable relief worker. For complex care participants, relief staff are trained on your specific requirements before supporting you.
Yes. We understand that life changes. Your support schedule can be adjusted as your needs, routines, and circumstances evolve — just speak to our team.
