Our nurses work closely with your GP, specialists, allied health professionals, and support workers to ensure your health needs are managed safely, consistently, and in the comfort of your own home.

What is Community Nursing?

Community nursing is a clinical support service delivered in your home or local community by qualified registered nurses (RNs) or enrolled nurses (ENs). Under the NDIS, community nursing is funded when a participant has complex health needs that require professional clinical oversight as part of their everyday life distinct from general personal care.

How it Works

Clinical needs assessment

A registered nurse will complete an initial assessment to understand your health needs, current supports, medications, and goals in your home, at a time that suits you.

Individualised care plan

We develop a clinical care plan in collaboration with your GP, specialist, and support coordinator ensuring all health needs are documented and communicated across your care team.

Regular nursing visits and review

Our nurses visit on a schedule matched to your clinical needs, with thorough documentation and regular reviews to adapt your care as circumstances change.

1. Is community nursing covered by my NDIS plan?

Community nursing is typically funded under Improved Health and Wellbeing or Improved Daily Living, depending on your individual circumstances. Your support coordinator or LAC can advise on your eligibility and how to include it in your plan.

2. Can a community nurse manage my medications?

Yes. Our registered nurses can administer and manage medications as prescribed by your treating doctor, including complex multi-medication regimes.

3. Will I always see the same nurse?

We prioritise consistency of care and aim to assign a primary nurse to each participant. A trained backup nurse is arranged to cover leave or unavailability, with proper clinical handover in place.

4. Can community nursing be combined with personal care?

Yes. Many participants receive both nursing support and personal care through Pacific Home Care Solutions, with all team members communicating regularly to ensure consistent, well-coordinated care.
