Pacific Home Care Solutions was founded in Victoria by two experienced professionals in the disability sector who shared a vision of creating support services that feel personal, compassionate, and genuinely centred around the individual. After working together for years and seeing gaps within traditional care models, it is about truly understanding the person; their goals, culture, routines, and what matters most to them.
With strong community trust and positive impact, Pacific Home Care Solutions has since expanded its services into Tasmania.

Today, our diverse and multilingual team includes experienced support workers, registered nurses, support coordinators, and allied health professionals delivering compassionate, high-quality support across daily living, community participation, mental health, Supported Independent Living, and complex care services.

How we got here
Pacific Home Care Solutions began as a small, locally focused team in Melbourne with a simple belief — those NDIS participants deserved support delivered with genuine compassion, consistency, and care. While we have grown steadily over time, we remain grounded in the same values and person-centred approach that shaped us from the very beginning.

Before we recommend any service or introduce any support worker, we take the time to truly understand you — your goals, preferences, daily life, and vision for independence. Our assessment process is thorough because the quality of everything that follows depends on it.
We match, not assign
We do not simply send whoever is available. We carefully match participants with support workers based on personality, cultural background, language, interests, and experience — because the right match is the foundation of meaningful support.
We work as one team
Our support workers, nurses, coordinators, and allied health partners work closely together to ensure your supports remain holistic, consistent, and never fragmented across different services.

With a Master of Social Work and over seven years of experience across the disability and aged care sectors, I founded Pacific Home Care Solutions with a passion for delivering supports that are genuinely personal, compassionate, and empowering. My experience as a Support Coordinator, Service Coordinator, Mental Health Support Worker, and Case Manager has allowed me to support individuals with diverse and complex needs while always promoting dignity, independence, and choice. I am deeply committed to walking alongside participants and families, ensuring they feel heard, valued, and supported every step of the way.

With a strong background in business, community services, and disability support, I founded Pacific Home Care Solutions with a vision to create a service that genuinely prioritises people, relationships, and quality care. Over the years, I have worked closely across the disability and mental health sectors supporting individuals with psychosocial disabilities, complex behaviours, and high-support needs while building services centred around dignity, independence, and trust. My experience leading support teams, coordinating care, and working directly alongside participants and families has shaped my belief that great support goes beyond simply providing a service it is about creating stability, connection, and meaningful outcomes for every individual. I remain deeply committed to ensuring every participant feels respected, understood, and empowered to live life on their own terms.
Our wider team of support workers and specialists
Beyond our leadership team, Pacific Home Care Solutions employs and coordinates a growing workforce of dedicated support workers, enrolled nurses, mental health practitioners, ILO coordinators, and community access staff across all four cities. Every team member undergoes rigorous screening, comprehensive induction training, and ongoing professional development to ensure the delivery of safe, high-quality, and person-centred support.
