Support Coordination is an NDIS Capacity Building support that helps you implement your NDIS plan and connect with services. It is funded separately from your Core Supports and focused on building your long-term capacity to manage your own supports.
Tier 1
For participants who need help understanding and implementing their NDIS plan. Covers connecting with providers, setting up service agreements, and resolving day-to-day issues that arise.
Tier 2
For participants with complex support needs who require a higher level of specialist coordination. Addresses more significant barriers and manages complex, multi-system challenges.
