Understanding the Support

What is Support Coordination?

Support Coordination is an NDIS Capacity Building support that helps you implement your NDIS plan and connect with services. It is funded separately from your Core Supports and focused on building your long-term capacity to manage your own supports.

Tier 1

Support Coordination

For participants who need help understanding and implementing their NDIS plan. Covers connecting with providers, setting up service agreements, and resolving day-to-day issues that arise.

Tier 2

Specialist Support Coordination

For participants with complex support needs who require a higher level of specialist coordination. Addresses more significant barriers and manages complex, multi-system challenges.

A good support coordinator works for you not for a provider, not for a system. Our job is to make your NDIS plan actually deliver what it was funded to deliver.

Frequently asked questions

1. Does my NDIS plan need to include Support Coordination?

Yes. Support Coordination is a funded NDIS support that must be included in your plan before it can be accessed. It is not automatically funded for every participant. Support Coordination may be included when a participant requires assistance to understand, implement, and manage their NDIS supports and build long-term capacity and independence.

2. Can my support coordinator also be my daily support provider?

Yes. Pacific Home Care Solutions can provide both Support Coordination and direct support services. However, our coordinators maintain a participant-focused approach and support participants to make informed choices about providers that best align with their goals, preferences, and support needs.

3. What is the difference between a Support Coordinator and a Local Area Coordinator (LAC)?

A Local Area Coordinator (LAC) is funded by the NDIA and primarily assists participants with accessing the NDIS, developing initial plans, and connecting with general community supports. A Support Coordinator is a funded support within an NDIS plan that provides more individualized and ongoing assistance to help participants implement, coordinate, and manage their supports effectively.

4. Can I change support coordinators if I am not happy?

Yes. Participants have the right to change support coordinators if they feel their current supports are not meeting their needs. Pacific Home Care Solutions can assist with a smooth transition process and work closely with participants, families, guardians, and existing providers to minimise disruption and maintain continuity of supports.
